Parental or Guardian Release Form

The form must be completed by a Minor's parents or legal guardian.  The definition of a minor is a student who is LESS than 18 years old when he/she begins UNT or IELI. 

This form is very important because a health provider (doctor, nurse, etc.) legally CANNOT provide any medical service to a sick child without the expressed written permission. 

Students must bring the completed form when coming to UNT.  Keep until requested by a medical professional.
